Pharmacy Practice Regulations 2005
The Governor in Council makes the following Regulations:

1. Objective
The objective of these Regulations is to prescribe
particulars and information relating to registration
under the Pharmacy Practice Act 2004.
2. Authorising provision
These Regulations are made under section 139 of
the Pharmacy Practice Act 2004.
3. Commencement
These Regulations come into operation on 1 July
2005.
4. Application for registration
For the purposes of section 4(2)(e) of the
Pharmacy Practice Act 2004, the prescribed
particulars that must be contained in an
application for registration are—
(a) the full name of the applicant;

(b) the details of other names the applicant uses
or has used;
(c) the date of birth of the applicant;
(d) the telephone number of the applicant.
5. Certificate of registration
For the purposes of section 21(2)(e) of the
Pharmacy Practice Act 2004, the prescribed
information that must be included on a certificate
of registration is—
(a) the full name of the registered pharmacist;
(b) the date the certificate was issued;
(c) the period the registration remains in force.
