No. 101/2004 Nurses Act 1993 Nurses Regulations 2004 The Lieutenant-Governor as the Governor's Deputy with the advice of the Executive Council makes the following Regulations: Dated: 3 August 2004 Responsible Minister: BRONWYN PIKE Minister for Health DIANE CASEY Clerk of the Executive Council 1. Objective The objective of these Regulations is to prescribe for the purposes of the Nurses Act 1993— (a) the forms of applications relating to registration or notation or endorsement of registration of a nurse; and (b) the particulars to be included in the register and certificates of registration. 2. Authorising provision These Regulations are made under section 93 of the Nurses Act 1993. 3. Definition In these Regulations "the Act" means the Nurses Act 1993. 5. Form of application for registration For the purposes of section 5(2)(a) of the Act, the prescribed form is the form set out in Schedule 2. 6. Form of application for notation of registration to practise Chinese medicine For the purposes of section 8A(5)(a) of the Act, the prescribed form is the form set out in Schedule 3. 7. Form of application for endorsement of registration as a Nurse Practitioner For the purposes of section 8B(4)(a) of the Act, the prescribed form is the form set out in Schedule 4. 8. Form of application for endorsement of registration of a Division 2 Nurse For the purposes of section 8C(3)(a) of the Act, the prescribed form is the form set out in Schedule 5. 9. The register For the purposes of section 17(3)(c) of the Act, the following information is the prescribed information— (a) the address of the person; (b) the person's professional qualifications; (c) the date of the person's initial registration; (d) the person's registration number. r. 4 -- 3 of 12 -- Victorian Legislation Parliamentary Documents S.R.
