No. 45/2004 Nurses Act 1993 Nurses (Amendment) Regulations 2004 The Governor in Council makes the following Regulations: Dated: 18 May 2004 Responsible Minister: BRONWYN PIKE Minister for Health STEPHANIE LENN Acting Clerk of the Executive Council 1. Objective The objective of these Regulations is to prescribe the form for application for endorsement of registration under section 8C of the Nurses Act 1993. 2. Authorising provision These Regulations are made under section 93 of the Nurses Act 1993. 3. New regulation 4C inserted After regulation 4B of the Nurses Regulations 1994 1 insert— "4C. Form of application for endorsement of registration of a division 2 nurse For the purposes of section 8C(3)(a) of the Act, the prescribed form is the form set out in Schedule 1C.". -- 2 of 5 -- Victorian Legislation Parliamentary Documents S.R. New Schedule 1C inserted After Schedule 1B to the Nurses Regulations 1994 insert— "SCHEDULE 1C Regulation 4C Nurses Regulations 1994 APPLICATION FOR ENDORSEMENT OF REGISTRATION OF A DIVISION 2 NURSE UNDER SECTION 8C OF THE NURSES ACT 1993 TO UNDERTAKE MEDICATION ADMINISTRATION To the Nurses Board of Victoria I apply for endorsement of registration under section 8C of the Nurses Act 1993 on the basis that: (a) I have satisfactorily completed a course of study in medication administration approved by the Board; or (b) I have satisfactorily completed a course of study or I have completed a qualification which, in the opinion of the Board, is substantially equivalent or is based on similar competencies to a course referred to in paragraph (a). * I am a registered nurse in Division 2 of the register of nurses kept under the Nurses Act 1993. * I have enclosed a separate application for registration as a nurse in Division 2 of the register kept under the Nurses Act 1993.
