Health Act 1958
Health (Registration of Premises) Regulations 2002
The Governor in Council makes the following Regulations:

1. Objective
The objective of these Regulations is to prescribe
matters relating to registrations under Part XIX of
the Health Act 1958.
2. Authorising provisions
These Regulations are made under sections 376,
390 and 391 of the Health Act 1958.
3. Commencement
These Regulations come into operation on
24 August 2002.
4. Revocation
The Health (Registration of Premises) Regulations
19921 are revoked.

5. Register book
For the purposes of section 371(1) of the Health
Act 1958, the prescribed particulars as to all
registrations, renewals of registrations or transfers
of registrations, other than those made under
Part XII of that Act, are—
(a) date of application for registration;
(b) name and address of proprietor;
(c) business or trading name (if any);
(d) address of premises;
(e) date of registration;
(f) registration number;
(g) dates of renewal of registration;
(h) date of transfer of registration (if applicable);
(i) conditions on the grant of registration,
renewal of registration or transfer of
registration (if applicable);
(j) name and address of person to whom
registration is transferred (if applicable);
(k) premises to which registration is transferred
(if applicable).
6. Certificates of registration
For the purposes of section 374(1) of the Health
Act 1958 a certificate of registration or renewal of
registration, other than under Part XII of that Act,
must be in the form set out in the Schedule.
