No. 133/2001 Chinese Medicine Registration Act 2000 Chinese Medicine Registration Regulations 2001 The Governor in Council makes the following Regulations: Dated: 4 December 2001 Responsible Minister: JOHN THWAITES Minister for Health HELEN DOYE Clerk of the Executive Council 1. Objective The objective of these Regulations is to prescribe information and forms relating to registration under the Chinese Medicine Registration Act 2000. 2. Authorising provision These Regulations are made under section 93 of the Chinese Medicine Registration Act 2000. 3. Commencement These Regulations come into operation on 1 January 2002. 4. Definition In these Regulations "the Act" means the Chinese Medicine Registration Act 2000. -- 2 of 6 -- S.R. No. 133/2001 Chinese Medicine Registration Regulations 2001 2 5. Application for registration For the purposes of section 4(2)(a) of the Act, the prescribed information that must be contained in an application for registration under the Act is— (a) the full name of the applicant; (b) the details of other names the applicant uses or has used (e.g. maiden name); (c) the date of birth of the applicant; (d) the telephone number of the applicant. 6. Form of application for endorsement of registration For the purposes of section 8(5)(a) of the Act, the prescribed form is set out in the Schedule. 7. The register In addition to the particulars set out in section 17(4) of the Act, the prescribed information to be included on the register is— (a) the date of the practitioner's initial registration; (b) the practitioner's registration number; (c) the details of whether the registration of the practitioner is general or specific. 8. Certificate of registration In addition to the particulars set out in section 18(2) of the Act, the prescribed information to be included on a certificate of registration is— (a) the full name of the registered practitioner; (b) whether he or she is registered as a Chinese herbal medicine practitioner, acupuncturist or Chinese herbal dispenser; (c) the date the certificate was issued; r. 5 -- 3 of 6 -- S.R. No. 133/2001 Chinese Medicine Registration Regulations 2001 4 SCHEDULE Regulation 6 Chinese Medicine Registration Regulations 2001 APPLICATION FOR ENDORSEMENT OF REGISTRATION UNDER THE CHINESE MEDICINE REGISTRATION ACT 2000 To the Chinese Medicine Registration Board of Victoria I apply for endorsement of registration under section 8 of the Chinese Medicine Registration Act 2000. * I am a registered Chinese medicine practitioner under the Chinese Medicine Registration Act 2000. * I am a registered Chinese herbal dispenser under the Chinese Medicine Registration Act 2000. * I enclose a separate application for registration as a *Chinese medicine practitioner/*Chinese herbal dispenser under the Chinese Medicine Registration Act 2000.
