No. 134/2001 Dental Practice Act 1999 Dental Practice (Amendment) Regulations 2001 The Governor in Council makes the following Regulations: Dated: 4 December 2001 Responsible Minister: JOHN THWAITES Minister for Health HELEN DOYE Clerk of the Executive Council 1. Objective The objective of these Regulations is to prescribe the form of an application for endorsement of registration for the purposes of section 8A of the Dental Practice Act 1999. 2. Authorising provision These Regulations are made under section 98 of the Dental Practice Act 1999. 3. Commencement These Regulations come into operation on 1 January 2002. -- 2 of 5 -- S.R. No. 134/2001 Dental Practice (Amendment) Regulations 2001 2 4. Amendment to the objectives In regulation 1 of the Dental Practice Regulations 20001 , after paragraph (a) insert— "(ab) the form of application for endorsement of registration of a dental care provider (other than a registered dental prosthetist) under section 8A of the Act; and". 5. New regulation 5A inserted After regulation 5 of the Dental Practice Regulations 2000 insert— "5A. Form of application for endorsement of registration to practise Chinese medicine For the purposes of section 8A(5)(a) of the Act, the prescribed form is set out in the Schedule.". 6. New Schedule inserted After regulation 7 of the Dental Practice Regulations 2000 insert— "SCHEDULE Regulation 5A Dental Practice Regulations 2000 APPLICATION FOR ENDORSEMENT OF REGISTRATION UNDER SECTION 8A OF THE DENTAL PRACTICE ACT 1999 To the Dental Practice Board of Victoria I apply for endorsement of registration under section 8A of the Dental Practice Act 1999 to practise Chinese medicine. * I am a registered dental care provider (other than a registered dental prosthetist) under the Dental Practice Act 1999. r. 4 -- 3 of 5 -- S.R. No. 134/2001 Dental Practice (Amendment) Regulations 2001 3 * I enclose a separate application for registration as a dental care provider (other than a registered dental prosthetist) under the Dental Practice Act 1999.
