No. 136/2001 Nurses Act 1993 Nurses (Amendment) Regulations 2001 The Governor in Council makes the following Regulations: Dated: 4 December 2001 Responsible Minister: JOHN THWAITES Minister for Health HELEN DOYE Clerk of the Executive Council 1. Objective The objective of these Regulations is to prescribe the forms for applications for notation and endorsement of registration for the purposes of the Nurses Act 1993. 2. Authorising provision These Regulations are made under section 93 of the Nurses Act 1993. 3. Commencement (1) These Regulations, except regulations 5 and 7, come into operation on the day after they are made. (2) Regulations 5 and 7 come into operation on 1 January 2002. 4. Amendment to the objective -- 2 of 7 -- S.R. No. 136/2001 Nurses (Amendment) Regulations 2001 2 In regulation 1 of the Nurses Regulations 1994 1 , for paragraph (a) substitute— "(a) the forms of applications relating to registration of a nurse;". 5. New regulation 4A inserted After regulation 4 of the Nurses Regulations 1994 insert— "4A. Form of application for notation of registration to practise Chinese medicine For the purposes of section 8A(5)(a) of the Act, the prescribed form is set out in Schedule 1A.". 6. New regulation 4B inserted Before regulation 5 of the Nurses Regulations 1994 insert— "4B. Form of application for endorsement of registration as a nurse practitioner For the purposes of section 8B(4)(a) of the Act, the prescribed form is set out in Schedule 1B.". 7. New Schedule 1A inserted After Schedule 1 to the Nurses Regulations 1994 insert— "SCHEDULE 1A r. 4 -- 3 of 7 -- S.R. No. 136/2001 Nurses (Amendment) Regulations 2001 3 Regulation 4A Nurses Regulations 1994 APPLICATION FOR NOTATION OF REGISTRATION UNDER THE NURSES ACT 1993 TO PRACTISE CHINESE MEDICINE To the Nurses Board of Victoria I apply for notation of registration under section 8A of the Nurses Act 1993 to practise Chinese medicine. * I am a registered nurse under the Nurses Act 1993. * I enclose a separate application for registration as a nurse under the Nurses Act 1993. PERSONAL DETAILS Title and name (Title) (Given Names) (Family Name) Any other names (e.g. maiden name) Address Telephone number Date of birth COURSE OF STUDY OR TRAINING I enclose the details of the course of study or training that I have completed which I submit qualifies me to practise Chinese medicine. I enclose the fee of (insert fee) Signature of applicant Date *Delete if inapplicable.". 8. New Schedule 1B inserted r. 7 -- 4 of 7 -- S.R. No. 136/2001 Nurses (Amendment) Regulations 2001 4 Before Schedule 2 to the Nurses Regulations 1994 insert— "SCHEDULE 1B Regulation 4B Nurses Regulations 1994 APPLICATION FOR ENDORSEMENT OF REGISTRATION UNDER THE NURSES ACT 1993 AS A NURSE PRACTITIONER To the Nurses Board of Victoria I apply for endorsement of registration under section 8B of the Nurses Act 1993 to— * use the title (insert category of nurse practitioner) nurse practitioner. * use the title (insert category of nurse practitioner) nurse practitioner and obtain and have in my possession and use, sell or supply the Schedule 2, 3, 4 and 8 poisons within the meaning of the Drugs, Poisons and Controlled Substances Act 1981 that are prescribed under that Act for that category of nurse practitioner. * I am a registered nurse under the Nurses Act 1993. * I enclose a separate application for registration as a nurse under the Nurses Act 1993. No. 136/2001 Nurses (Amendment) Regulations 2001 5 * the details of the course of study that I have completed and the clinical experience that I have undertaken which I submit qualify me to use the title (insert category of nurse practitioner) nurse practitioner. * the details of— (i) the course of study that I have completed and the clinical experience that I have undertaken which I submit qualify me to use the title (insert category of nurse practitioner) nurse practitioner; and (ii) the course of study that I have completed which I submit qualifies me to obtain and have in my possession and to use, sell or supply the Schedule 2, 3, 4 and 8 poisons within the meaning of the Drugs, Poisons and Controlled Substances Act 1981 that are prescribed under that Act for that category of nurse practitioner.
