Regulations 2001
The Governor in Council makes the following Regulations:

1. Objective
The objective of these Regulations is to prescribe
the form of application for notation of registration
for the purposes of section 8A of the
Optometrists Registration Act 1996.
2. Authorising provision
These Regulations are made under section 92 of
the Optometrists Registration Act 1996.
3. Commencement
These Regulations come into operation on
1 January 2002.

4. Amendment to the objectives
In regulation 1 of the Optometrists Registration
Regulations 1997 1 , for paragraph (b) substitute—
"(b) the forms of application for notation and
endorsement of registration under that
Act; and".
5. New regulation 4A inserted
After regulation 4 of the Optometrists Registration
Regulations 1997 insert—
"4A. Form of application for notation of
registration to practise Chinese medicine
For the purposes of section 8A(5)(a) of the
Act, the prescribed form is set out in
Schedule 1A.".
6. New Schedule 1A inserted
After Schedule 1 to the Optometrists Registration
Regulations 1997 insert—

APPLICATION FOR NOTATION OF
REGISTRATION UNDER SECTION 8A OF THE
OPTOMETRISTS REGISTRATION ACT 1996
To the Optometrists Registration Board of Victoria
I apply for notation of registration under section 8A of the
Optometrists Registration Act 1996 to practise Chinese
medicine.
* I am a registered optometrist under the Optometrists
Registration Act 1996.
* I enclose a separate application for registration as an
optometrist under the Optometrists Registration
Act 1996.
