No. 34/2000 Dental Practice Act 1999 Dental Practice Regulations 2000 The Governor in Council makes the following Regulations: Dated: 30 May 2000 Responsible Minister: JOHN THWAITES Minister for Health HELEN DOYE Clerk of the Executive Council 1. Objectives The objectives of these Regulations are to prescribe— (a) the information to be contained in an application for registration as a dental care provider under the Dental Practice Act 1999; and (b) the information to be included on the register and in registration documents issued under that Act. 2. Authorising provision These Regulations are made under section 98 of the Dental Practice Act 1999. 3. Commencement These Regulations come into operation on 1 July 2000. -- 2 of 4 -- S.R. No. 34/2000 Dental Practice Regulations 2000 2 4. Definition In these Regulations, "the Act" means the Dental Practice Act 1999. 5. Application for registration In addition to the matters set out in section 4(2) of the Act, the prescribed information to be contained in an application for registration as a dental care provider is— (a) the title and full name of the person; (b) the details of any other name used by the person; (c) the date of birth of the person; (d) the telephone number of the person; (e) a statement as to whether the registration sought is general or specific. 6. The register In addition to the matters set out in section 16(6) of the Act, the prescribed information to be included on the register is— (a) the person's qualifications; (b) the date of the person's initial registration; (c) the person's registration number; (d) the details of whether the registration of the person is general or specific. 7. Evidence of registration For the purposes of section 17(1) of the Act, the prescribed information to be included on a written document issued as evidence of registration is— (a) the name of the person registered; (b) the date the document was issued; r. 4 -- 3 of 4 -- S.R. No. 34/2000 Dental Practice Regulations 2000 3 (c) the period the registration remains in force; (d) whether the registration is general or specific; (e) the division and (where applicable) the subdivision of the registration granted; (f) in relation to a registered dentist, whether the dentist has been registered as a dental specialist in a special branch of dentistry; (g) any condition, limitation or restriction imposed on the registration of the person. ═══════════════ r. 7 -- 4 of 4 --
