No. 77/1997 Chiropractors Registration Act 1996 Chiropractors Registration Regulations 1997 The Governor in Council makes the following Regulations: Dated: 22 July 1997 Responsible Minister: ROB KNOWLES Minister for Health SHARNE BRYAN Clerk of the Executive Council 1. Objectives The objectives of these Regulations are to prescribe— (a) the form of application for registration as a chiropractor under the Chiropractors Registration Act 1996; (b) the particulars to be included in the register and certificates of registration under that Act. 2. Authorising provisions These Regulations are made under sections 4, 15, 16 and 88 of the Chiropractors Registration Act 1996. 3. Definition In these Regulations— -- 2 of 4 -- S.R. No. 77/1997 Chiropractors Registration Regulations 1997 2 "the Act" means the Chiropractors Registration Act 1996. 4. Form of application for registration For the purposes of section 4(2)(a) of the Act, an application for registration as a chiropractor must be in the form set out in the Schedule. 5. The register For the purposes of section 15(3)(c) of the Act, the following information is the prescribed information to be included in the register— (a) the address of the registered chiropractor; (b) his or her professional qualifications; (c) the date of his or her initial registration; (d) his or her registration number; (e) details of any cancellation or suspension of registration. 6. Certificate of registration For the purposes of section 16(2)(b) of the Act, the following information is the prescribed information to be included on a certificate of registration— (a) the name of the person registered; (b) his or her qualifications; (c) the date the certificate was issued. __________________ r. 4 -- 3 of 4 -- S.R. No. 77/1997 Chiropractors Registration Regulations 1997 3 SCHEDULE APPLICATION FOR REGISTRATION Regulation 4 PERSONAL DETAILS Title and name of the applicant Any other names by which the applicant is known (e.g. maiden name) The address of the applicant The telephone number of the applicant The date of birth of the applicant PROFESSIONAL QUALIFICATIONS Details of the qualification or qualifications in chiropractic held by the applicant The name of the institution or institutions which awarded the qualification or qualifications The length of time normally taken to complete the course on a full- time or part-time basis The date the applicant commenced the course The date the applicant completed the course If previously registered as a chiropractor— (a) the date and place of initial registration (b) the date and places of any other registrations ═══════════════ Sch. -- 4 of 4 --
