No. 110/1997 Health Act 1958 Health (Certificate of Analysis) Regulations 1997 The Governor in Council makes the following Regulations: Dated: 21 October 1997 Responsible Minister: ROB KNOWLES Minister for Health SHARNE BRYAN Clerk of the Executive Council 1. Objective The objective of these Regulations is to prescribe the form of a certificate of analysis for the purposes of the Health Act 1958. 2. Authorising provisions These Regulations are made under sections 275, 390(1) and 391 of the Health Act 1958. 3. Revocation The Health (Certificates of Analysis) Regulations 19871 are revoked. 4. Certificate of analysis For the purposes of section 275 of the Health Act 1958, the certificate of analysis supplied by an analyst must be in the form in the Schedule. __________________ -- 2 of 4 -- S.R. No. 110/1997 Health (Certificate of Analysis) Regulations 1997 2 SCHEDULE Regulation 4 Health Act 1958 Health (Certificate of Analysis) Regulations 1997 CERTIFICATE OF ANALYSIS I (insert full name of analyst) of (insert address of analyst) a person approved under the Health Act 1958 to undertake this type of analysis, certify— 1. That on (insert date) I received from (insert name and address of person submitting sample) a sample— *sealed, with seal intact, bearing the impression (insert description of impression) *with seal broken *unsealed and marked (insert description of marking, if any) 2. That I have analysed the sample and that the result of my analysis is as follows— (insert full analytical result) 3. *I am of the opinion that no change has taken place in the constitution of the sample which would affect the proportion of any constituent set out above.
