No. 78/1997 Optometrists Registration Act 1996 Optometrists Registration Regulations 1997 The Governor in Council makes the following Regulations: Dated: 22 July 1997 Responsible Minister: ROB KNOWLES Minister for Health SHARNE BRYAN Clerk of the Executive Council 1. Objectives The objectives of these Regulations are to prescribe— (a) the form of application for registration as an optometrist under the Optometrists Registration Act 1996; (b) the form of application for the endorsement of registration under that Act to use certain Schedule 4 poisons; and (c) the particulars to be included in the register and certificates of registration under that Act. 2. Authorising provisions These Regulations are made under sections 5, 11, 17, 18 and 92 of the Optometrists Registration Act 1996. -- 2 of 6 -- S.R. No. 78/1997 Optometrists Registration Regulations 1997 2 3. Definition In these Regulations— "the Act" means the Optometrists Registration Act 1996. 4. Form of application for registration For the purposes of section 5(2)(a) of the Act, an application for registration as an optometrist must be in the form set out in Schedule 1. 5. Form of application for endorsement of registration For the purposes of section 11(3)(a) of the Act, an application for endorsement of the registration of an optometrist that he or she is qualified to obtain and to have in his or her possession and to use, sell or supply certain Schedule 4 poisons within the meaning of the Drugs, Poisons and Controlled Substances Act 1981 is in the prescribed form if it sets out the information in the order set out in Schedule 2. 6. The register For the purposes of section 17(3)(d) of the Act, the following information is the prescribed information to be included in the register— (a) the address of the registered optometrist; (b) his or her professional qualifications; (c) the date of his or her initial registration; (d) his or her registration number; (e) details of any cancellation of registration. 7. Certificate of registration For the purposes of section 18(2)(c) of the Act, the following information is the prescribed information to be included on a certificate of registration— r. 3 -- 3 of 6 -- S.R. No. 78/1997 Optometrists Registration Regulations 1997 4 SCHEDULE SCHEDULE 1 APPLICATION FOR REGISTRATION Regulation 4 PERSONAL DETAILS Title and name of the applicant Any other names by which the applicant is known (e.g. maiden name) The address of the applicant The telephone number of the applicant The date of birth of the applicant PROFESSIONAL QUALIFICATIONS Details of the qualification or qualifications in optometry held by the applicant The name of the institution or institutions which awarded the qualification The length of time normally taken to complete the course on a full- time or part-time basis The year the qualification was awarded The name of the course undertaken to obtain that qualification If previously registered as an optometrist— (a) the date and place of initial registration (b) the date and places of any other registrations ————————— Sch. 1 -- 5 of 6 -- S.R. No. 78/1997 Optometrists Registration Regulations 1997 5 SCHEDULE 2 APPLICATION FOR ENDORSEMENT OF REGISTRATION Regulation 5 Title and name of the applicant The address of the applicant The telephone number of the applicant Details of the course of study which qualify the applicant to obtain and to have in his or her possession and to use, sell or supply certain Schedule 4 poisons within the meaning of the Drugs, Poisons and Controlled Substances Act 1981, including— (a) the name of the institution or institutions at which the course was undertaken; (b) the title of the course; (c) the length of time normally taken to complete the course on a full-time or part-time basis; (d) the date the course was commenced; (e) the date the course was completed. ═══════════════ Sch. 2 -- 6 of 6 --
